BELOW CAREFULLY


  • If this is your first time logging into the Teamsters Local 667 self-enrollment system, please follow the instructions found under FIRST TIME USER.
  • If you already have a username and permanent password, follow the instructions under RETURNING USERS.

 FIRST TIME USER:

  • Start by clicking "Login/ Register" on the top right of your screen.
  • In the "NEW APPLICANT" box on the left, please click CONTINUE.
  • On the form enter your, First Name, Last Name and  Email Address (Your Email Address will become your permanent user name) 
  • Next, choose a permanent password and enter it into the registration form, twice.
  • A verification email will be sent to you. Please open it and click on the verification link. 

RETURNING USERS:
  • Start by clicking "Login/ Register" on the top right of this screen.
  • Please enter the user name and password that you created the first time you signed in.
  • Your email address is your username.  
  • If you don't have your password, please use the "FORGOT PASSWORD" button to assign a new password.

AFLAC GROUP ACCIDENT

After an accident, you may have expenses you’ve never thought about. Can your finances handle them?   • Pays a wellness benefit per person per year. $25 for year 1; $50 for years 2-4; $75 for year 5 and after.


AFLAC GROUP HOSPITAL

Aflac’s group Hospital Indemnity insurance pays cash benefits that can be used to help with those out-of-pocket hospital expenses that may not be fully covered by major medical insurance.

  • Benefits for hospital stays and expenses

  • Benefits for hospital admission, confinement, intensive care and intermediate intensive care step-down unit

  • Health Screening Benefit of $50 per person per year


AFLAC GROUP CRITICAL ILLNESS

The Aflac group Critical Illness insurance plan can help with the treatment costs of covered critical illnesses, such as Cancer, a heart attack, or a stroke. More importantly, the plan helps you focus on recuperation instead of the distraction and stress over the costs of medical and personal bills. With group Critical Illness insurance from Aflac, you receive cash benefits directly (unless otherwise assigned) giving you the flexibility to help pay bills related to treatment or to help with everyday living expenses.


Benefits:

  • Lump-sum benefit for a covered critical illness such as: Cancer, heart attack, stroke, major organ transplant, and end-stage renal failure

  • Health Screening Benefits of $50 per person per year.


TRANSAMERICA SHORT TERM DISABILITY
