If you are Currently employed by Lee Health and this is your first time signing in to the Lee Health benefits self enrollment system, please follow the instructions below under FIRST TIME USER.  If you already have a user name and password, see RETURNING USERS below.

FIRST TIME USER:  
If your username can not be found by our system, follow these instructions...
  • Start by clicking "Login/ Register" at the top right of your screen.
  • In the "NEW APPLICANT" box on the left, please click CONTINUE.
  • On the form enter your, First Name, Last Name and  Email Address (Your Email Address will become your permanent user name). 
  • Next, choose a permanent password and enter it into the registration form, twice.
  • A verification email will be sent to you. Please open it and click on the verification link. 

    • If you are still unable to sign in, send us a message using the CHAT box on the lower right of your screen.  Live chat is available Monday - Friday 8am-6pm EST.  If you are trying to enroll outside of our support hours, leave us a message though the chat system.  Someone will get back to you when support resumes.   Please include your name, email address and phone number.


    RETURNING USERS:
    • Start by clicking "Login/ Register" on the top right of this screen.
    • Please enter the user name and password that you created the first time you signed in.
    • Your email address is your username.  
    • If you don't have your password, please use the "FORGOT PASSWORD" button to assign a new password.

    Group Accident Insurance

    After an accident, you may have expenses you’ve never thought about. Can your finances handle them? It’s reassuring to know that an accident insurance plan can be there for you through the many stages of care, from the initial emergency treatment or hospitalization, to follow-up treatments or physical therapy.     Group Critical Illness Insurance

    The Aflac group Critical Illness insurance plan can help with the treatment costs of covered critical illnesses, such as Cancer, a heart attack, or a stroke. More importantly, the plan helps you focus on recuperation instead of the distraction and stress over the costs of medical and personal bills. With group Critical Illness insurance from Aflac, you receive cash benefits directly (unless otherwise assigned) giving you the flexibility to help pay bills related to treatment or to help with everyday living expenses.


    Benefits:

    • Lump-sum benefit for a covered critical illness such as: Cancer, heart attack, stroke, major organ transplant, and end-stage renal failure

    • Health Screening Benefits of $50 per person per year and $200 per year for a mammogram.
